This document explains the use of telehealth for your care with Lighthouse EverLucent Health. By booking and attending a visit, you acknowledge that you have read and agree to the terms below.
What telehealth is
Telehealth is the delivery of clinical care using real-time video, audio, and secure messaging instead of an in-person visit. Your visits and chart are delivered through Optimantra, a HIPAA- compliant EMR and video platform. Ongoing messaging is handled through Spruce Health. Email from the practice is sent encrypted through Paubox.
Benefits
- Convenient access without travel.
- Direct, longer visits with the same provider every time.
- Faster follow-up on labs and medication adjustments.
Risks & limitations
- A physical exam is not possible. Some conditions cannot be diagnosed or treated safely without an in-person visit, lab work, or imaging.
- Technology can fail. Video or audio may freeze, disconnect, or have poor quality.
- Despite HIPAA-compliant systems and encryption, no electronic transmission is 100% secure.
- In rare cases, a lack of access to your complete medical history could affect clinical judgment.
If technology fails
If your visit disconnects, I will attempt to reconnect by video, then by phone using the number on file. If I cannot reconnect within a reasonable time, I will reschedule at no additional charge.
Not for emergencies
Telehealth is not appropriate for emergencies. If you are experiencing chest pain, suicidal thoughts, severe bleeding, signs of stroke, or any other emergency, call 911 or go to the nearest emergency department.
Location & licensure
You must be physically located in a state where Mallory Jones, MSN, APRN, FNP-C, CWHS is licensed at the time of each visit. You will be asked to confirm your location at the start of each appointment. Care cannot be delivered if you are outside a licensed state.
Your rights
- You may withhold or withdraw consent at any time, in writing.
- You may request an in-person referral if telehealth is not appropriate.
- All laws and ethical standards that apply to in-person care also apply to telehealth, including confidentiality and access to your records.
- You have the right to a copy of this consent and of your medical record on request.
Recording
Visits are not recorded without your written consent. Clinical notes are documented in Optimantra as part of your medical record.
