Lighthouse EverLucent Health prescribes controlled substances (including testosterone used in TRT care) only when clinically indicated and only in accordance with federal law, DEA rules, and the laws of the state where the patient is located. This policy explains how I evaluate, prescribe, and monitor these medications.
Eligibility
- You must be 18 years of age or older.
- You must be physically located in a state where Mallory Jones, MSN, APRN, FNP-C, CWHS is licensed and where telehealth prescribing of the specific medication is permitted.
- A complete intake, history, and clinical evaluation is required before any controlled substance is prescribed. I do not prescribe on a first-contact basis without an evaluation.
Identity verification & PDMP
Government-issued photo ID is verified at intake. Before each controlled-substance prescription, I check the state Prescription Drug Monitoring Program (PDMP) to review your controlled-medication history. Findings inconsistent with safe care may lead me to decline prescribing.
One prescriber, one pharmacy
- You agree to receive controlled-substance prescriptions for the treated condition only from Lighthouse EverLucent Health while you are an active patient.
- You will fill these prescriptions at one designated pharmacy. Changing pharmacies requires prior notice.
- Lost, stolen, or early-refill requests are evaluated on a case-by-case basis and are generally not replaced.
Follow-up & monitoring
- Regular follow-up visits are required to continue controlled- substance therapy. Skipping required follow-ups will pause refills.
- Random or scheduled drug screening, additional labs, or in-person evaluation may be required at the provider's discretion.
- You agree not to share, sell, or divert any prescribed medication.
What I do not prescribe via telehealth
- Opioid pain medications for chronic pain.
- Benzodiazepines for long-term daily use.
- Any controlled substance to a patient outside the states where Mallory is licensed, or where state or federal law restricts telehealth prescribing of that substance.
Grounds for discontinuation
I may discontinue controlled-substance therapy and/or end the provider-patient relationship for reasons including, but not limited to: positive findings of non-prescribed substances, evidence of diversion, obtaining controlled substances from other providers without disclosure, repeated missed visits, abusive behavior toward staff, or any conduct I judge unsafe.
