Under the federal No Surprises Act, you have the right to receive a Good Faith Estimate ("GFE") of expected charges for medical services if you are uninsured or choose not to use insurance. Lighthouse EverLucent Health is a cash-pay practice and does not bill insurance, so this notice applies to every patient.
What the GFE includes
- The visit or membership you are scheduling.
- The expected charge for that visit or membership.
- Reasonably expected items or services from this practice over the next 12 months (for example, follow-up visits in a membership).
The GFE does not include charges from outside vendors — labs, imaging, and pharmacies bill you separately and will provide their own estimates on request.
When you'll get it
- When you schedule a visit at least 3 business days in advance — within 1 business day of scheduling.
- When you schedule at least 10 business days in advance — within 3 business days of scheduling.
- Any time you request one in writing.
Dispute resolution
If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill through the federal Patient-Provider Dispute Resolution process. You must start the dispute within 120 calendar days of the date on your original bill.

Email hello@lighthouseeverlucenthealth.com with the visit or membership you are considering and I will send your estimate in writing.
Keep a copy of this notice and your Good Faith Estimate in a safe place. You may need them if you are billed a higher amount.
