Perimenopause: The Decade No One Warned You About
Why women can feel "off" for years before menopause — and why "your labs look fine" usually misses it.

Perimenopause is one of the most under-recognized phases of a woman's life, and one of the most consequential. Patients arrive in their early 40s — sometimes their late 30s — describing months or years of feeling "off." They've usually been told it's stress, anxiety, aging, or that their labs "look fine."
The problem is that perimenopause is not defined by hormone loss. It's defined by hormonal instability. And a single lab draw is a terrible tool for measuring instability.
What perimenopause actually is
Perimenopause is the transition between the reproductive years and menopause. It begins when ovarian signaling becomes less predictable — long before periods stop.
The hormonal pattern is messy on purpose:
- Estrogen fluctuates — often with higher highs and lower lows than your 20s.
- Progesterone declines earlier and more steeply than estrogen.
- Testosterone drifts down gradually.
- Cortisol sensitivity increases — stress hits harder.
- Insulin resistance worsens, sometimes subtly.
Menopause is the destination (12 consecutive months without a period). Perimenopause is the road, and it can be 5–10 years long.
Early signs women — and clinicians — often miss
The classic story is hot flashes and night sweats. Those are real, but they tend to arrive later. The earlier signals usually involve sleep, mood, cognition, and metabolism:
- Sleep that's "lighter" than it used to be, or waking at 3 a.m.
- Anxiety that feels chemical — out of proportion to what's happening
- Brain fog, word-finding pauses, a sense of being a step slower
- Cycles that are still mostly regular but feel different — heavier, shorter, more painful
- Sudden weight gain around the midsection despite no real change in lifestyle
- Loss of libido, vaginal dryness, or pain with intercourse
- Joint aches that have no orthopedic explanation
- Fatigue that sleep doesn't fully fix
None of these are specific to perimenopause on their own. The pattern is what matters — and the pattern is what gets missed when each symptom is treated in isolation.
Why "your labs look fine" misses it
Standard labs in perimenopause often look unremarkable, for two reasons:
- Hormone levels fluctuate hour to hour and day to day. A single FSH or estradiol level is a snapshot of a moving target. A "normal" reading on a Tuesday tells you very little about Thursday.
- Reference ranges are population-based. A "normal" testosterone for a 45-year-old woman is not necessarily a functional testosterone for her.
The clinical question isn't "are your numbers inside the bell curve?" It's "do your symptoms, your cycles, and your labs — together — tell a coherent story?"
That's not a question that gets asked in a six-minute visit.
What helps — long before periods stop
There is no single treatment for perimenopause because perimenopause is not a single thing. A real plan usually combines several layers:
Foundation Sleep, protein, resistance training, blood sugar stability. These are not consolation prizes — they're the substrate everything else works on top of.
Targeted hormonal support, when appropriate
- Progesterone is often the first intervention because it tends to decline first. Oral micronized progesterone at bedtime can meaningfully improve sleep and reduce anxiety for many patients.
- Estradiol (typically transdermal) can address vasomotor symptoms, sleep, mood, vaginal symptoms, and bone protection.
- Testosterone in low physiologic doses can support libido, energy, mood, and lean mass — and the evidence is stronger than most women have been told.
Adjacent care Thyroid evaluation, iron and ferritin status, vitamin D, and a metabolic panel including fasting insulin. Perimenopause amplifies whatever else is already off.
What about safety?
The Women's Health Initiative casts a long shadow. Two decades later, the reinterpretation matters:
- For most healthy women in or near menopause, modern bioidentical hormone therapy is safe and effective when individualized.
- Transdermal estradiol carries a different (lower) clot risk than oral conjugated estrogens.
- Risks and benefits should be discussed individually, not absorbed from a headline.
That conversation is part of the visit, not a footnote.
You don't have to wait it out
The most common thing patients tell me after I start treatment is some version of: "I forgot what feeling like myself was supposed to feel like."
That's the version of healthcare I want to practice. Not "your labs look fine." Not "this is just what your 40s feel like." A real conversation about what's actually happening in your body, and what I can do about it now.
