Lighthouse EverLucent Health (“the Practice”) is a cash-based telemedicine medical practice committed to protecting the privacy and security of personal information and protected health information (“PHI”) in accordance with applicable federal and state laws, including the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).

Scope of Services

  • All medical services are provided via telemedicine

  • All patients must complete an initial video visit to establish care

  • Patients must be physically located in the state in which they reside and where the provider is licensed at the time of the visit

  • Payment for a visit does not guarantee that any medication or treatment will be prescribed

  • Clinical decisions are based solely on medical appropriateness

  • The Practice is cash-only and does not bill insurance

  • An optional concierge membership may be offered; participation is not required to receive care

Electronic Medical Record & Patient Portal

All medical records and clinical documentation are maintained in OptiMantra, a HIPAA-compliant electronic medical record (EMR) system.

Patients may access the patient portal:

  • Through a secure link on the Practice website, or

  • Directly through OptiMantra’s web or mobile application

The patient portal is the primary method for submitting clinical information, completing intake forms, and communicating with the provider.


Communications & Secure Messaging

  • Spruce Health is used for HIPAA-compliant secure messaging, calls, and follow-up communication

  • Paubox is used for HIPAA-compliant encrypted email when PHI is included

  • Patients are encouraged not to submit medical information through the general website contact form


Website Contact Form

The website includes a general contact form intended for administrative or scheduling inquiries only.
Patients should not submit sensitive medical or health information through this form. Clinical information should be submitted only through the secure patient portal.


Cookies & Website Technologies

The website may use cookies or similar technologies to support basic website functionality or to understand general usage patterns. These technologies do not collect medical information or PHI.

Use & Disclosure of PHI
