By accessing or using the services of Lighthouse
EverLucent Health, you agree to these Terms of
Service. If you do not agree, please do not use
our services. - SERVICES PROVIDED
Lighthouse EverLucent Health provides telehealth
consultations and ongoing care through Mallory
Jones, FNP-BC. Services are provided via secure
video visits and are available in 42 states.
All services are 100% virtual. - CASH-PAY PRACTICE
Lighthouse EverLucent Health does not accept
insurance. All services are self-pay. Payment is
due at the time of service or membership enrollment. - MEMBERSHIP TERMS
Membership fees are billed monthly. You may cancel
your membership at any time. Cancellations must be
submitted in writing to
info@lighthouseeverlucenthealth.com prior to your
next billing date. No refunds are issued for
partial months. - NOT AN EMERGENCY SERVICE
Lighthouse EverLucent Health is not an emergency
medical service. If you are experiencing a medical
emergency, call 911 or go to your nearest
emergency room immediately. - PRESCRIPTION POLICY
Prescriptions are issued at the sole clinical
discretion of Mallory Jones, FNP-BC. No medication
is guaranteed. Prescriptions are only issued when
clinically appropriate. - PATIENT RESPONSIBILITIES
You agree to provide accurate and complete health
information, disclose all medications and
supplements, follow up as recommended, and notify
us of any changes in your health status. - LIMITATION OF LIABILITY
Lighthouse EverLucent Health is not liable for
outcomes resulting from incomplete or inaccurate
patient-provided information, failure to follow
treatment recommendations, or circumstances beyond
