Where Do I Even Start with Fertility? A Step-by-Step Plan

Trying to get pregnant can feel overwhelming fast.

There’s so much information online, so many opinions, and often no clear plan. Most people end up bouncing between providers, ordering random labs, or waiting longer than they should.

If you’re not sure where to start, here’s a simple, real-world approach.

Step 1: Know When to Get Help

You don’t need to wait forever.

  • Under 35 → try for 12 months
  • 35+ → try for 6 months
  • Over 40 or known issues → don’t wait

This is the general medical guideline for when to start an evaluation.

Step 2: Start with the Basics (Not Everything at Once)

You don’t need every test right away. Start with the ones that actually matter:

For Women:

  • Hormone labs (timed correctly)
  • Ovulation confirmation
  • Thyroid + metabolic labs

For Men:

  • Semen analysis (this is often skipped but shouldn’t be)

👉 Male factor contributes to ~40% of infertility cases—it’s not just a female issue.

Step 3: Understand What’s Being Evaluated

Fertility evaluation looks at 3 main things:

  1. Are eggs being released? (ovulation)
  2. Are sperm healthy?
  3. Can egg and sperm meet? (tubes + uterus)

These are the core drivers of conception.

Step 4: Add Imaging If Needed

If labs look okay but pregnancy isn’t happening:

  • Ultrasound → looks at ovaries + uterus
  • HSG → checks if fallopian tubes are open

These help identify structural issues early instead of guessing.

Step 5: Address the “Hidden” Factors

This is where a lot of people get missed.

Things that impact fertility:

  • Insulin resistance
  • Thyroid dysfunction
  • Inflammation
  • Weight + metabolic health

Even if labs are “normal,” these can still affect ovulation and egg quality.

Step 6: Decide Next Steps (Not Everyone Needs IVF)

Once you have data, you can actually make decisions:

  • Keep trying naturally
  • Ovulation support
  • IUI
  • IVF referral (if needed)

👉 Not everyone needs aggressive treatment—but waiting too long without a plan can delay things.

Step 7: Don’t Try to Piece This Together Alone

This is the biggest issue I see.

Most patients are:

  • Ordering labs in different places
  • Getting partial answers
  • Trying to connect everything themselves

That’s exhausting—and unnecessary.

What I Do Differently

I help you:

  • Order the right labs (timed correctly)
  • Coordinate imaging
  • Review everything together
  • Create a clear plan
  • Connect you to the right next step (if needed)

So you’re not guessing.

Bottom Line

Fertility doesn’t have to be confusing.

You don’t need everything at once—you just need the right steps in the right order.
